**Important: LatteZone requires signature of Agreement stating that you hereby authorized

Helio membership agreement within 121 days from your activation date.
Also Helio will charge you a penalty fee of early termination of $175
if you terminate your service before the agreed contract term of 2 years.

4. PROCEDURES (What to Expect)
1. After the customer purchases the phone through LatteZone’s online website, follow the steps below in order to activate the device.
2. Customer needs to fill out and sign the Lattezone Agreement.
3. Customer needs to send the forms by fax (408-452-0802) or email (acastro@lattezone.com)
4. LatteZone will contact Helio to check the customer’s credit.
5. LatteZone may need to collect $0 up to $250 refundable deposit from the customer. This depends on the credit approval from Helio.

6. LatteZone activates the device.
7. LatteZone ships out the product to the customer using Fedex Exress (You will be provided with a tracking #).
